Model Release Disclosure
USE OF YOUR PHOTOGRAPHIC IMAGE
Robertson’s Martial Arts Photography, Inc (RMAPCO) would
like to use your image for purposes listed below, but not limited to these
purposes alone.
 | RMAPCO sample books. These books are brought to photo shoots,
tournaments, conventions and various events. The books are used to
showcase the photography and artwork that has been created by RMAPCO.
| RMAPCO web site. This site is a showcase of RMAPCO products and services.
| RMAPCO advertising. From time to time we may create magazine ads for submission
to various publications. We may create brochures, postcards, business cards or
other various forms of advertising materials.
| RMAPCO Online store. We may sell Photo composites and artwork in the
form of, but not limited to, 8x10 photos, large format exhibit prints,
collector’s trading cards, etc. (The subject may receive a 10% model fee on items sold.)
| Martial Arts Industry. From time to time we may furnish various martial
arts companies with photographic images. Some of the companies we have submitted images to
in the past are: Automated Payment Systems (APS), Educational Funding Company (EFC),
Martial Arts Industry Association (MAIA), National Association Of Martial Arts
Professionals (NAPMA), and Samurai Sports. These photos have appeared in books,
magazines, ad slicks, posters and a variety of other mixed media.
| Because this release is signed does not guarantee that your image will be used.
| If you would prefer to have your image made available for specific purposes and
not made available for other purposes, please mention this in the comments portion
of this form.
| If for whatever reason there is a martial arts company, organization, etc. that
you do not want your images released to please make note in the comments section
of this form.
| We may not be able to notify you if or when your image is used. Sometimes the
images are printed months after we have released them, and we are not made aware
of the fact.

MODEL RELEASE
I do hereby give Robertson’s Martial Arts Photography Inc. The irrevocable right to use my
name and these photographs for reproduction in any print or electronic medium for purposes
of advertising, sell, trade, display, exhibition, competition or editorial use. I have read
this release and understand and agree to its terms. I affirm that I am at least 18 years
of age or am the legal guardian of the above named student.
